You can now self-refer for NHS physiotherapy. The system has changed in recent times and you can no longer phone.You need to complete a form and either email it or send it off by post. Please click here for the form or collect from the front desk in surgery.



Sussex Community NHS Trust Physiotherapy Service wants to make it easier for patients to get advice from a Physiotherapist. Previously you would need to see you GP before you could be referred for Physiotherapy.


However, we are trying a new approach whereby you can self refer to a Physiotherapist.


How does it work?

If you have a back, neck, joint or general muscle problem you can complete the self referral form and send to the MSK team.


Once you have completed the form you can send it to:-

Or drop it tinto Bognor War memorial physio dept - please note, you won't be issued an appointment at the time of dropping it off.

Who can use this service?

The service is NOT appropriate for the following patients:


  • Under 16 years of age
  • Those suffering from multiple or complex musculoskeletal conditions
  • Those with complex medical histories
  • pregnant women
  • Those with poor grasp of the English language
  • Those who have learning difficulties
  • Those who have hearing difficulties
  • Those who are housebound

If you are in any of the above categories, please ask you GP to make a written referral instead.
